What it actually is
Eyebrow hair doesn't grow in one direction. At the head of the brow it points upward; along the body it sweeps outward; at the tail it angles down. Every graft has to be placed at the angle that suits its position, lying almost flat against the skin.
Only single-hair follicular units are used — a two-hair graft in an eyebrow is noticed instantly. The result is a slow procedure with a low graft count but a very high placement workload.
Because the donor hair comes from the scalp, transplanted eyebrow hair keeps growing and needs trimming roughly every two to three weeks. That's a permanent maintenance commitment, and we discuss it before, not after.

The most unforgiving few hundred grafts in surgery
The eyebrow is a small area holding an enormous amount of information. Hairs at the head of the brow point upward, sweep sideways along the body and angle downward at the tail, lying almost flat against the skin throughout. A single graft at the wrong angle is visible from across the room — in a way a misplaced graft in a four-thousand-graft scalp never is.
Everything is single-hair. Density is deliberately kept below what a natural brow carries, because transplanted hair is thicker than native eyebrow hair and an over-dense brow looks drawn on rather than grown.
The maintenance nobody mentions
Transplanted eyebrow hair keeps the growth cycle of the scalp it came from, which means it doesn't stop at eyebrow length. It needs trimming — typically every two to three weeks, permanently. Some patients also find that in the first months the hair needs occasional training to lie in the desired direction.
This isn't a complication; it's the deal itself — and it's the thing patients most wish they had understood before the appointment. That's why a good eyebrow consultation spends as much time on maintenance as on design.
Why some eyebrow cases are declined
Not every thin brow is a surgical problem. Loss from over-plucking is usually the ideal indication, because the follicles have been exhausted mechanically, not by disease. Loss caused by thyroid disease, alopecia areata or a scarring condition such as frontal fibrosing alopecia is an entirely different matter, and transplanting into an active inflammatory process wastes grafts and can make the area worse.
The other common reason to decline is expectation. A transplanted eyebrow is grown hair with a scalp growth cycle: it needs regular trimming for life, takes twelve months to settle and will never have the uniform edge of microblading or a drawn-on brow. Patients comparing it with cosmetic tattooing are comparing two different products, and it's better to say so before surgery than after.
