What a hair transplant costs and what you are actually paying for.

Nobody can give you a real number from a photograph — but anyone can explain how the number is built. Here is the arithmetic behind a hair transplant quote in Istanbul and abroad, including the parts of a low price that get paid for later.

The short version

In Türkiye, hair transplants are mostly priced as packages and the market range is wide: around €1,500–2,500 for packages at high-volume clinics with hotel and transfers included, €2,500–4,500 in the mid-market, and clearly above that at clinics where the surgeon personally performs the extraction and implantation — often €5,000 and beyond. In Europe and the US the same procedure is priced at roughly €/$3–12 per graft, which puts a 2,500 graft case in the €/$10,000–25,000 band.

These are market ranges, published so you can sanity-check any quote you are given. They are not our quote. At Azrak Hair the number comes after an assessment, because the two inputs that determine it — the density of your donor area and the size of the area to be covered — are measurements, not guesses from a phone photograph.

The six things that actually move the price

Two quotes for the same head can differ threefold, and the difference is almost never the quality of the sapphire blade. It is these six variables.

  • Graft count. The single largest factor. More grafts means more extraction time, more channel creation and more implantation labor — which is why an honest clinic will not inflate the number, and a package operation will.
  • Who performs the surgery. In high-volume clinics the surgeon designs the hairline and technicians perform the extraction and implantation. Surgeon-performed extraction and implantation costs more, because it consumes a full surgical day for one patient.
  • Patients per day. A clinic running four to six patients a day through one surgeon has a fundamentally different cost base — and a fundamentally different amount of attention to spare per graft — than a clinic that takes a single case.
  • Technique. DHI with Choi implanters is generally priced above Sapphire FUE because it is slower and more labor-intensive per graft, not because it is inherently superior. Unshaven work costs more again for the same reason.
  • Where the clinic is. Rent, wages, insurance and licensing costs in Istanbul are not the same as in London or New York. Most of the international price gap is this, not surgical skill.
  • What follow-up is included. A single follow-up call is cheap. A twelve-month program with scheduled photographic reviews costs the clinic real time, and it is the part that gets quietly removed from a discount price.

Non-surgical treatments are priced per session, not per graft. PRP and mesotherapy courses are usually sold as a series of three to six sessions, and are frequently packaged with surgery to support the native hair the transplant does not treat.

Why the cheapest quote is usually the most expensive

The failure mode at the discount end of this industry is well documented, and it is always the same. A fixed package — often “up to 5,000 grafts” — is sold to a patient whose donor area cannot safely fund it. To hit the number, extraction is spread across the whole back of the head. The result grows in acceptably at month twelve, and the donor area looks visibly thin the first time the patient asks for a short haircut.

That cannot be undone. Extracted follicles do not grow back, and no procedure restores donor density. A revision case — correcting pluggy grafts, a hairline placed too low or an over-harvested donor — routinely costs more than the original operation and delivers less, because it works with whatever the first surgeon left behind. The cheapest quote is therefore not a price; it is a bet on your donor area.

Package clinic vs surgeon-led clinic

Istanbul is the world’s largest hair transplant center, and hundreds of thousands of patients come here every year. The best clinics here are genuinely excellent; the cheapest build their price on volume. The comparison below is not a comparison of cities but of two business models — and in the same city, a few streets apart, you can find both.

The honest comparison is not procedure price against procedure price. It is the total cost, plus the value you place on being examined before you commit, and on the operating surgeon being reachable in month six when the crown is growing more slowly than the front and you want to know whether that is normal.

What a quote should include

Ask for the quote in writing and ask specifically which of the following it includes. The gap between two prices is often entirely in this list.

  • Consultation and pre-operative assessment, including donor density measurement.
  • Blood tests and any pre-operative screening the clinic requires.
  • The procedure itself, anesthesia, and the surgical team's time for the whole day.
  • Post-operative medication and the take-home aftercare kit.
  • The first wash, performed or supervised by the clinic.
  • Scheduled check-ups and photographic follow-up — ask how many, and over how long.
  • A written policy on what happens if growth is poor: what is reviewed, when, and at what cost.
  • PRP or supporting treatment sessions, which are frequently priced separately.

Payment and installments

A hair transplant is an elective procedure; in Türkiye it is not covered by SGK or by private health insurance and is paid out of pocket. Patients commonly finance it through credit card installments, a personal bank loan or the clinic’s own payment plan. Each has a real cost: installment surcharges on cards, interest and arrangement fees on loans, and early repayment or cancellation penalties on some plans.

If a clinic offers a payment plan, ask three things before signing: how much more the total cost is than the upfront price, what happens to the amount you have paid if you cancel or postpone the procedure, and whether the contract is with the clinic or with a third party that will keep holding the debt whatever your result.

Pricing red flags

None of these proves a clinic is bad. Each deserves a direct question, and a clinic worth having surgery with will answer without changing the subject.

  • An exact price and graft count given from photographs, before any examination.
  • A discount that expires within days, or a price that drops when you mention you are considering another clinic.
  • A guaranteed graft survival percentage, or guaranteed density.
  • A package that charges for grafts you do not need, with no reduction if fewer are used.
  • Follow-up priced as an optional extra.
  • No written breakdown of what the price includes.
  • A quote from someone who has never told you your donor density in follicular units per square centimeter.

Common questions

Why does no clinic publish a single price?
Because the two things that set the price — how many grafts your donor can safely give and how large an area you want covered — can only be measured in person. A single published price is a package, and a package is priced for the average patient, not for you.
Is a hair transplant covered by SGK or private insurance?
Not when it is done for androgenetic alopecia; in Türkiye it counts as a cosmetic procedure and is not covered by SGK, the Turkish state health insurer. Reconstruction after burns, trauma or surgical scarring is sometimes assessed differently — that decision belongs to your insurer, not to the clinic.
Why are the price differences so large?
The difference mostly comes from who performs the procedure. A clinic running six to eight patients a day through technician teams and a clinic where the surgeon devotes the whole day to one patient are not selling the same product. Labor, facilities and technique also move the price, but the biggest variable is how the surgical hours are divided.
Should I pay per graft or a fixed package?
Neither structure is disqualifying on its own. Per-graft pricing rewards extracting more grafts; fixed packages reward selling the largest package. What matters is whether your graft count came from an examination or from a price list.
Do I need to pay a deposit to hold a date?
Deposits are normal practice. Pressure to pay before you have been examined, or a discount that expires this week, is a sales technique, not a scheduling requirement.
