Almost all the anxiety around this operation is made of unanswered questions. Here are the answers, in order — from the moment the line is drawn to the month the result finally arrives.
“What if it doesn't suit my face?”
Your hairline is designed on you — standing, upright, in natural light — and you approve it in a mirror before consent is signed. It follows your facial proportions and your age, not a template. A line that looks right at 35 has to still look right at 55.
“Will I be put under? Will it hurt?”
The scalp is numbed with local anesthetic in stages. There is no general anesthetic and no intubation. The injections are the uncomfortable part and they last a few minutes; after that you can talk, eat, and watch something for the rest of the day.
“Are they going to cut my head open?”
No strip is cut. A micromotor punch under a millimeter wide removes each follicular unit individually from the donor zone at the back and sides. Thousands of tiny extractions rather than one long incision — that is the whole difference between FUE and the older strip method.
“What does DHI actually mean?”
A Choi implanter pen holds the follicle inside a hollow needle. The needle enters the scalp and a plunger releases the graft at depth — one motion instead of two. Less time out of the body, and the surgeon sets the angle for every single graft.
“Will I be left with a scar?”
FUE leaves hundreds of dot-shaped marks under a millimeter across, spread out so no single area is depleted. Once the surrounding hair grows to a few millimeters they are not visible. There is no linear scar — that belongs to the strip technique.
“Is it going to look fake?”
Hair does not grow straight out of the scalp. It exits at an angle and in a direction that changes across the head — forward at the front, swirling at the crown. Each graft is placed to match. Get this wrong and it reads as artificial from across a room, no matter how many grafts went in.
“How bad will I look?”
Expect swelling for two to three days, then small crusts around each graft that clear within about ten days. Redness fades over two to four weeks and lingers longer on pale skin. Most patients are comfortable in public somewhere around day ten to fourteen.
“Did I just waste my money?”
Between weeks two and six, most transplanted hairs shed. The follicle stays — only the shaft is lost, while the follicle enters a resting phase. It is the single most alarming stage of the process and the most predictable. Nothing has gone wrong.
“When will I actually see something?”
New growth typically starts around month three to four, fine and sparse. By month six there is visible coverage. Between months nine and twelve the hair thickens and matures. Final judgment belongs at month twelve to eighteen, not before.
“Will it just fall out again?”
Follicles from the back and sides are genetically resistant to DHT, the hormone that drives pattern loss. Moved to the front, they keep that resistance. What they do not do is protect your remaining native hair — which is why a plan for the hair you still have matters as much as the surgery.
“How many grafts do I need?”
A graft is a follicular unit containing one to four hairs. So 3,000 grafts is usually somewhere around 6,000 to 7,500 hairs. Clinics that quote hair counts instead of graft counts are making the number look bigger. Ask which one you are being quoted.
